CincomSmalltalkWiki: Survey Application Page

I have written a truly simple survey application using the VisualWave Web Toolkit. This application consists of a servlet for taking the survey, and an SSP page for reporting survey results. The SSP pages and application setup files can be downloaded here (seems broken, try here).

The code can be found in the public repository, Survey package version 1.1 (or later). The package and individual classes are commented; you should be able to get going fairly quickly.
